I think that number is NOT the Model number of the card, but rather some manufacturer's part or lot number. The Model number of the cards does not usually appear on the cards, as I recall. The 12 dB cards have a row of resistors and a row of capacitors. The 18 dB cards are somewhat more complicated-looking than that.

Of course writing in the center will result in the frequency being obscured by the front panel since it's window is offset....
