Where do I get the correct sonotube enclosure?
You might find some ideas here on the Acoustic isolation chamber for LE5-series thread.
Once you get the tube, use the Jasper Circle Jig to cut a plywood plug of exact diameter, and stuff in into one end of the tube - glue it tight. And, don't forget to leave the plug slightly skewed in the tube...

Go to an electrical supply house adn buy a small section of 5 inch pvc pipe. It is Grey and works perfectly. Home Depot and Lowe's don't carry it.
You can use gorilla glue to secure a slug of wood in the back

You can see the 5 inch PVC coupler I used. THe coupler is for joining two 5 inch pipes together. With a 3/4 inch particle board plug it came out at 89 cu inches of volume, just about perfect for the midrange.
